Q.What does the admission process entail?

The admission process starts at the hospital bedside. We want to cultivate a relationship with you and your loved one. It is essential to find out your needs, likes and dislikes and to answer any questions you may have up front. Our Care Managers will continue to have daily communication with you and your case manager. Avante always want to keep you apprised of where we are in the admission process: I.e., approval, insurance authorization, discharge orders, and scheduled transfer date and time. We are with you every step of the way. We want your transition to be as smooth and comfortable as possible. Once in our facility, you will be greeted with a warm welcome and excellent customer service.

Q. How often will my physician come to see me when I am at your facility?

Our rounding physicians are required to assess patients upon admissions. Not only do they come to the facility, but they have a team of nurse practitioners, physicians’ assistants, and rounding registered nurses who go and see pts at the bedside. We also utilize telemedicine at our facilities to ensure optimum care of our patient and quality outcomes.

Q. What should residents bring to an Assisted Living, Memory Care, or Skilled Nursing Facility?

Assisted Living and Memory Care are new residences, and we would want you to bring some of your home to ours. Clothing, accessories, toiletries, wall art, pictures, keepsakes, and bedding helps make the new environment a familiar one. Residents can personalize their rooms to show ownership and independence. Assisted living may offer more space for additional furniture, depending on the size room that you choose.
Due to the nature of memory care, fewer items can offer a more natural transition with decreased confusion regarding the change in the environment. Pictures, blankets, clothing, and natural toiletries are a great start to building a comfortable environment.
Skilled Nursing is often a short term stay with increased medical attention. Since this is a therapeutic environment, a resident’s medical needs dictate what additional items can be brought.

Q. Can residents have a private room?

Private and semi-private rooms are available. When speaking with admissions, ask about what would be the best option regarding room choice.

Q. What does Assisted Living, Memory Care, and Skilled Nursing going to Cost?

All three levels of care will have different costs based upon the requirement of care. Often, insurance will cover the majority of the cost of services. Call us today and learn more about your options for payment.

Q. Do you accept insurance?

Yes. We are in-network with many insurance companies. Depending on specific needs, insurance may apply to the payment.

Q. Can residents have private aid?

Absolutely! If you have a private aid that you already work with, he or she can also come to our facility; it can make the transition easier for you or your loved one. We also offer additional care plans if you feel that your loved one needs extra support at either assisted living, memory care, or skilled nursing facilities.

Q. Do residents need their own car?

We include transportation to shopping, doctors’ appointments, social outings, and so on. All residents are welcome to take advantage of our transportation services, and this can also eliminate costs such as car payments and insurance.

Q. Will residents have assistance with medication?

Yes. We understand that medication can sometimes be a tedious task to remember. We make it our priority to make sure all residents stay medication compliant.

Q. What arrangements to consider when moving in?

When you move to an Avante location, all of your needs are taken care of in your monthly payments. Included are electric, cable, Wi-Fi, meals, and housekeeping. It is suggested that if in Assisted Living or Skilled Nursing that you keep your cell phone with you.

Q. When can residents have visitors?

At Avante, we are family and acknowledge how essential visits can be for all levels of care. We encourage your regular visits. Get to know the staff that your loved one is interacting with daily and increase trust and comfort with our services.

Q. Are meals provided onsite?

We provide three gourmet meals per day and snacks in-between meals.

Q. What is the staff to resident ratio?

Avante works hard to maintain government regulations on staffing. We are dedicated to quality care and outcomes. Therefore, we staff appropriately meet each patients need.

Q. How much therapy will my loved/I receive?

Our therapists are trained to meet each patient at their level of function and need. Therapy can be 4-6 days a week, depending on the initial evaluation and progression will continue from there

Q. Are pets allowed in the facility for visits?

Yes, indeed. At Avante, we recognize that our four-legged loved ones are a part of the family. Please be sure to have their shot records and appropriate documentation available upon visiting.
